问题

DEV_AGENT.md 缺少一条关键规则：创建委托/研究 Issue 后必须立即在原 Issue 设置 blocked 标签和阻塞评论链接。#75→#90 的阻塞关系是事后才补的。

修复

在 DEV_AGENT.md 的 blocked Issue 处理部分，补充原子操作要求：

  1. 创建委托 Issue（test-code/investigation）后立即在原 Issue 评论"阻塞: #新Issue"
  2. 立即给原 Issue 加上 blocked 标签
  3. 这两步和创建 Issue 是原子操作
