pzhang_zywl f2b9301fa1 fix: step1 覆盖反馈重试从 1 次增加到最多 2 次 - Closes #54

首次重试修复完路径/格式问题后,如果覆盖率仍不达标,追加第二轮重试
以进一步补充缺失的功能单元,降低 QE Audit inadequate_ratio。

LLM 生成的 source 有时缺少 section 字段,导致 Layer A schema 验证失败。
在 _normalize_rule 中添加防御性处理:从兄弟 source 或 rule path 推断 section。
