问题

#57 修复后 Layer B 覆盖率已恢复至 90.7% ✅，但仍有：

  1. Layer A: rules[56].sources 为空数组，违反 schema 要求
  2. Layer C: QE Audit 100% 不合格 (0/5 adequate)
